Immunizations will be offered by the City of Lubbock next week. The City health department will offer state supplied childhood immunizations for children ages 2 months to 18 years at the Patterson Library on Tuesday from 3 to 6 p.m. Medicaid and CHIP are accepted, and a complete shot record is required. Without a shot record, no immunizations will be given.

Birth records are available from the City of Lubbock for kids heading to school. The City’s Vital Statistics division will be selling birth records to parents and legal guardians, also at the Patterson Library on Tuesday from 3 to 6 p.m. The cost is $23, and must be paid in cash or money order. Those applying for birth records must have either a valid driver’s license or two documents proving identity, such as a social security card.

The Patterson Library is located at 1836 Parkway Drive.
